Children must be 13 years or younger at the time of enrollment to qualify for child pricing. Whitening procedures such as in-office whitening, at home bleaching trays, bleaching gel, or whitening pens, Oral B toothbrushes, brush head refills, and toothpastes sold in-office are not included in the plan discount. This is not an insurance plan and cannot be used in conjunction with any other insurance plan, dental plan or offer, or financing company, such as Care Credit. Plan is only redeemable at Eastern Hills Dental and will not be accepted at any specialists’ office to which you are referred. To receive the preferred discount, plan must be paid in full and full payment for any procedure is due at the time of treatment. Plan pricing is effective July 1, 2019 – June 30, 2020, or until your membership expires, which is one calendar year after enrollment. Any unused benefits cannot be refunded, credited, or carried over..
